How to configure and modify Grafana 9 alert template

I am new to Grafana version 9, I have setup a alert template to microsoft team like below:

I really just need a basic template with instance name on the tilte and the alert value below, but i can not find where to edit. Can anyone help or suggest me? Thank everyone.
