Panel - Create form and send email

Hi All
We use Grafana enterprise and created a custom home page for the whole ORG.
we have more than 75 data sources and i was thinking if its possible to create a panel with a form to submit data , like send an email ?
it would be used to send an email to our DevOps with a request to get access to a specific data source.
Let me know what you think.
