SSO in Grafana using Apache reverse proxy use custom login form

as per http://docs.grafana.org/tutorials/authproxy/#apache-basicauth I made the respective configuration in Apache httpd.conf file. But using this, Apache throws its default alert popup to enter credentials when i redirect to Grafana.

Instead of the dirty popup, i want to use my own custom login form. I used mod_auth_form like this:

But I am not able to redirect to Grafana by sending X-WEBAUTH-USER in headers
